Revascularization and largest free autograft restructuring in different biomechanical conditions replacement of defects of long bones (experimental research)

In the result of the experiments on 81 dogs (4 series of experiments) the revascularization and rebuilding of big free autotransplantants used for substitution of the top half of the tibia defect under compression and distraction by lengthening of the bony fragments according to Ilizarov’s method were studied with the help of roentgenovasographic, roentgenologic, densimometric and morphologic methods. The authors have shown and proved a real significance of biochemical factors on the outcome of revascularization and adjustment of free metadiaphizaric autotransplantants . In the process of distractional osteosynthesis unlike compressional osteosynthesis the favourable conditions for realization of compensatory and regenerative possibilities of the organism were created. They provided osteoreparation, revascularization with the restoration of blood supply of the transplanted bone and its organotypical reorganization. Reorganization of metadiaphizaric autotransplantant with lack of the factor of distraction took place with focus destruction of the bone far from the zone of accretion and was accompanied by lowering (OD) ( optical density ) of the bony tissue not more than 11,0+2,3 (level of critical value was considered 0,010). Insignificant amplitude of oscillations of densimometric index OD showed very slow development of rebuilding processes and rapid their extinction after accretion. It was verified morphologically.
